Our Model of Care

Our Approach to Supported Living in Milton Keynes & Buckinghamshire

We believe the best supported living is built on partnership and respect. Our approach puts each person at the centre. We plan with people, not just for them, to help adults live safely and independently at home. Every stage, from assessment to review, is focused on dignity, inclusion, and the outcomes that matter most.

How We Plan and Deliver Support

We use a clear, four-stage process to ensure every person gets personalised, responsive care across Milton Keynes, Buckinghamshire, and nearby areas.

Assessment

We start by getting to know you and your circumstances—understanding your health, preferences, strengths, and what independence means to you. We listen to your goals, family views, and professional advice.

Care Planning

Together, we co-create a care and support plan that reflects your wishes, risk assessments, and input from family, friends, or advocates. Plans are practical, person-centred, and flexible for change.

Support Delivery

Care is delivered by our trained, DBS-checked team—always with dignity, respect, and attention to your changing needs. We focus on what you can do, not just what you need help with.

Regular Review

Your plan is reviewed with you and your support network at regular intervals. We check that goals are met, feedback is acted on, and your care always fits your life.

Building Confidence & Everyday Skills

Supporting Independence in Daily Life

We help adults in Milton Keynes and Buckinghamshire build confidence, routines, and skills for independent living. Our support is person-centred, practical, and always focused on real outcomes.

Promoting Choice

We encourage people to make informed decisions about their routines, social life, and future goals.

Supporting Daily Routines

Gentle support with personal care, meals, and household tasks—always respecting independence and preferences.

Building Life Skills

Helping develop skills for budgeting, healthy eating, travel, and day-to-day organisation.

Encouraging Community Access

Support to join in with volunteering, hobbies, local activities, and faith or cultural groups.

Flexible, Evolving Support

Care plans adapt as needs and goals change—from short-term help to building long-term skills.

Measuring Real Outcomes

We track progress in independence, well-being, and confidence—using feedback from each person and their support network.

Safeguarding & Wellbeing

We make safeguarding everyone’s responsibility—from recruitment to incident management and ongoing governance. Our culture of safety and transparency ensures every person, family member, and professional feels protected and respected.

Putting Safety First

Safety and trust are our top priorities. Every team member is DBS checked, receives ongoing safeguarding training, and understands their role in protecting the people we support. Our policies and procedures meet all CQC and local authority standards.

Robust Governance & Transparency

We use regular audits, supervision, and open reporting so that concerns are addressed promptly and honestly. Families and commissioners can be confident we have clear processes for escalation, whistleblowing, and learning from incidents.

Community, Choice & Social Inclusion

We believe people thrive when they feel connected. Our approach reduces isolation, builds confidence, and promotes a real sense of belonging for every person we support.

Supporting Community Connections

We help adults stay involved in their local community—whether that means joining activities, visiting places of worship, volunteering, or building confidence in public. Reducing isolation is at the heart of our work.

Promoting Belonging & Choice

Our team supports each person to access local resources, connect with others, and make choices that reflect their interests, culture, and personal goals. Everyone deserves to feel they belong.
